Identify the device without collecting passcodes

Record model, IMEI or serial, colour and visible condition. The form does not include a password field. Instead, record whether the customer will unlock the device in person for testing.

Test functions before you open the phone

Mark each function Works, Not working or Not tested. Cover screen and touch, cameras, microphone, speaker, buttons, charging, biometric unlock, signal and the water indicator when the shop checks it.

Record data, warranty and water-damage acknowledgements

Confirm the customer’s backup statement and the shop’s reviewed data, warranty and water-damage acknowledgements. The template is an operational starting point, not jurisdiction-specific legal language.

Confirm how the customer wants updates

Record the permitted contact channel and the next useful checkpoint. For mail-in jobs, also record shipment references and parcel handoffs.

Common questions

Should a repair shop write down the customer passcode?

This template does not collect it. Arrange for the customer to unlock the device in person when testing requires access.

What if a function cannot be tested?

Mark it Not tested and record why. Do not imply that an unchecked function worked at intake.
